Schedule home visits and be on call duty accordingly to roster.
Prepare and write care plans for new patients.
Conduct monthly checks and replenishment of medication/medical supply and record controlled drugs.
Act as case manager.
Collaborate with members of a multi-disciplinary team.
Assess nutritional needs of patient with advanced diseases.
Identify patients and families who need financial assistance and refer to MSW/Welfare department.
Prepare family caregivers for approaching death.
Job Requirements
:
Knowledge in Basic Palliative Care Nursing.
Min 1 year palliative experience and 3 years of nursing experience.
Town area.
5days week, Office hours (830am to 6pm).
Salary: from $3,400 onwards + Allowance
